1) Click on the blue Add Files Button below and navigate to the file (.jpg or .png).

2) Click on the filename and the photo will be immediately uploaded.

3) You can upload as many as 5 photos on any form submission.

4) Click on "Send a copy to yourself" if desired.

5) Click on Send Button to complete the submission.
